本人现申请成为香港美容从业联会会员。本人同意遵守香港美容从业联会之组织章程、附则及一切经会员大会及理事会通过之决议。本人明白香港美容从业联会有权拒绝接纳本人之申请而毋须给予任何原因。
I hereby formally apply for the membership of HKBIU. I agree to abide by the constitution, supplementary articles and all the resolutions adopted by the committee meetings and board of directors. I understand that the HKBIU has the right to reject my application for membership without giving any reasons.
本人同意谨此声明，提供以上资料出于自愿，且真实、完整及准确无误。倘有更改，当主动告知香港美容从业联会。
I hereby confirm that the information above is true, complete, accurate and voluntarily given. If there is any amendment, I will take the initiative to inform the HKBIU.
申请人须知
香港美容从业联会收集的个人资料将作为审批会籍资格、会员服务、选举及联络用途(包括会讯及提供货品、设施活动及服务讯息、研究调查，或其他直接有关的活动，又或有关索求捐赠或贡献的讯息)。所有会员资料将予以保密。会员可要求查阅该资料及要求改正该资料的权利；如申请人欲在资料的使用上另有安排，或除必要通知外不希望接收其他资讯，可电邮通知本会(info@hkbiu.org)。
The personal information collected by the HKBIU will be used for the approval for membership, member services, elections and contact (including the provision of the HKBIU goods, the news about the union, events, facilities, services, surveys and the request for donations and contribution to the union). All the personal information of our members will be kept confidential. Members have the right to request checking and amending the information. If the applicant would like to request for special arrangement on the usage of personal information, or refuse to receive any news except the essential one, he or she may inform us via email (info@hkbiu.org).
