本人現申請成為香港美容從業聯會會員。本人同意遵守香港美容從業聯會之組織章程、附則及一切經會員大會及理事會通過之決議。本人明白香港美容從業聯會有權拒絕接納本人之申請而毋須給予任何原因。
I hereby formally apply for the membership of HKBIU. I agree to abide by the constitution, supplementary articles and all the resolutions adopted by the committee meetings and board of directors. I understand that the HKBIU has the right to reject my application for membership without giving any reasons.
本人同意謹此聲明，提供以上資料出於自願，且真實、完整及準確無誤。倘有更改，當主動告知香港美容從業聯會。
I hereby confirm that the information above is true, complete, accurate and voluntarily given. If there is any amendment, I will take the initiative to inform the HKBIU.
申請人須知
香港美容從業聯會收集的個人資料將作為審批會籍資格、會員服務、選舉及聯絡用途(包括會訊及提供貨品、設施活動及服務訊息、研究調查，或其他直接有關的活動，又或有關索求捐贈或貢獻的訊息)。所有會員資料將予以保密。會員可要求查閱該資料及要求改正該資料的權利；如申請人欲在資料的使用上另有安排，或除必要通知外不希望接收其他資訊，可電郵通知本會(info@hkbiu.org)。
The personal information collected by the HKBIU will be used for the approval for membership, member services, elections and contact (including the provision of the HKBIU goods, the news about the union, events, facilities, services, surveys and the request for donations and contribution to the union). All the personal information of our members will be kept confidential. Members have the right to request checking and amending the information. If the applicant would like to request for special arrangement on the usage of personal information, or refuse to receive any news except the essential one, he or she may inform us via email (info@hkbiu.org).
